Dr. Anna C. Peoples, PharmD, RPh, CDE

Peoples Foundation
Certified Diabetes Educator; CDC-recognized DSMES provider; ADCES-accredited; directs diabetes self-management education, DPP programming, and pharmacy-based chronic disease management services for underserved Hampton Roads residents.

Dr. Zarron Moses, MD
Board Co-Chair
Primary care physician and founder of Avail Medical Practice; serves as clinical referral partner for uninsured and underinsured participants accessing the Foundation’s care coordination and chronic disease management services.
Keyzia Nikole

Banking professional and nonprofit business consultant; oversees financial sustainability strategy, grant compliance, and nonprofit business development supporting program scalability and long-term access to care.

Qualified Mental Health Professional; Certified Community Health Worker; leads behavioral health integration, harm reduction outreach, Narcan training, and community health worker navigation services for program participants.

While Peoples Foundation is an emerging nonprofit, its leadership brings a proven track record of execution through Peoples Pharmacy and direct commercial real estate activity in the same market as PHWC.
2005 – Present
Commercial Property Acquisition and Management
Leadership has acquired and managed a commercial property in Hampton Roads, demonstrating the real estate acquisition, management, and long-term asset stewardship competencies required for the PHWC project. This is the same team that will execute PHWC.
Active commercial asset under continuous ownership and management for 20 plus years.
2016 – Present
Peoples Pharmacy: Community Healthcare Enterprise
Peoples Pharmacy has served the Hampton Roads community as a neighborhood pharmacy and trusted health resource. Its patient relationships, clinical programming, and community trust form the foundation for the PHWC concept.
Established community health brand with active patient relationships serving medically underserved residents.
2018 – Present
Peoples Foundation: Nonprofit Formation and Programming
Formally incorporated in 2018, Peoples Foundation has maintained continuous IRS compliance, built institutional partnerships, and developed the organizational infrastructure for health-focused community development.
501(c)(3) public charity with active programming and grant management experience.

PHWC Predevelopment: Active Project Execution
Site identified and in active negotiation with the City of Chesapeake. Partnership discussions underway with anchor health providers. Architectural plans in progress. Capital stack being structured with mission-driven lenders and public agencies.
